  This is an online E log book to discuss our patient's de-identified health data shared after taking his/her/guardian's signed informed consent. Here we discuss our individual patient's problems through series of inputs from available global online community of experts with an aim to solve those patient's clinical problems with collective current best evidence based inputs. Chief complaint:- A 26 years old female presented with the chief complaints of  - fever - heavy menstrual bleeding History of presenting illness:- Patient was apparently  asymptomatic 5 days back then she developed fever which of is low grade not associated with chills and rigor , relieved on medication.2 days back she had her menstrual bleeding which is high for which he went to hospital and was given medication. When she went for follow up check up it is found that her platelet count is low and dengueNS1 is reactive. History of past illness:-  N/K/c/o :- Diabetes mellitus , hypertension,TB,asthma

Chief complaint:- A 61 years old male presented with the chief complaints of  -pain in the left loin -decreased urine output -Swelling in the legs -Constipation and fever History of presenting illness:- Patient was apparantly asymptomatic 15days back. Then he had 1 episode of vomiting followed by fever associated with chills and rigors. Then he developed pain in the left loin associated with decreased urine output since 15days.
